Application Process
Students are referred by the home school districts through the localities' Family Assessment and Planning Teams (FAPT), with the consent of the parents or legal guardians. The FAPT team is responsible for funding the placement. Minnick Education Center does not accept private pay; therefore, individual families do not incur the cost of the child's education. Information required determining the students' eligibility/admission must be provided by the LEA and parent/legal guardian, and include completion of the Application Packet, which requires the following additional information:
- Signed FAPT release (please ensure that the FAPT release lists Minnick Education Centers)
- Most recent eligibility components to include minutes*
- Current IEP
- Immunization record
- Most recent physical

Other standardized testing*

Transcript or grade report

Transcript analysis signed by guidance counselor indicating courses taken and coursework needed (to include verified credit analysis)

Most recent report card (please include grade summary if student is admitted mid-grading period)

* This information is used to create specific educational and behavioral programming for the student.
Minnick Education Center staff will review the information after it is received. Upon application acceptance, an assessment visit is scheduled at MEC for the parents and the students. Parent and student will be interviewed and will have an opportunity to visit with staff to ask questions. The decision regarding enrollment is made following the assessment visit. If the student enrolls, it is the responsibility of the sending school district to complete all remaining paperwork relevant to the student's enrollment.
